Tony Tan, a former aide of Kok whose party membership was terminated in 2007, claimed he has submitted proof to the ROS over alleged acts by Kok that were said to have violated the party's constitution.
Tan was joined by DAP member Danny Leong Eng Tiom in the trip to the ROS on Tuesday.
The two men had last week submitted documents to the Malaysian Anti-Corruption Commission (MACC) on the alleged misappropriation of RM186,000 in election funds by Kok.
"According to the Party Constitution Clause 19, the PLC (party liaison committee) should meet at least once a year but the Seputeh PLC failed to hold any meeting at all.
"Each time when a meeting is called, the quorum is not enough.
"The Party Constitution Clause 25 clearly stated that petty cash drawn from the bank should not exceed RM400 each time, but she (Kok) has withdrawn RM8,000 once and RM5,000 three times from the bank," Tan said in a list of violations allegedly committed by Kok.
Tan had been in the news several times to hurl similar accusations against his former employer, who is the DAP national vice-chairperson.
Kok told The Star Online last week that such allegations from Tan and Leong were "nothing new" to her as they had been attacking her since she terminated Tan's service in 2006 after just nine months.
Kok alleged that since the termination, Tan had tried to take revenge by disturbing her in the 2008 and 2013 general elections.
Kok had last month demanded an apology and RM10mil in damages from Leong for tarnishing her reputation by accusing Kok in the media for misappropriation of funds.
